• Scroll right down to the foot of the page and you will see ‘click here to download printable form (requires microsoft word)‘
• This application form can be downloaded, completed and saved.

If you wish your application to be assessed, verified and then confirmed at the next Panel Meeting on 8th January 2009 you will need to submit the application by:  12th November 2008 .

For most providers electronic applications will be a speedy process:

• The Agency takes emailed applications and processes them.

• Christ Church C of E Primary School, North Somerset, Learning and Teaching through an Innovative Curriculum
• Education Walsall, Walsall, Strategic Partners providing educational support for looked after children through a Virtual School in partnership with the students and their carers.
